In consideration my engagement as participant/player by Prism Communications, the producer, on terms stated, I hereby give the right and permission for use of my recorded voice and photographic pictures through any media by the producer, in perpetuity.  I waive any right to inspect or approve the finished copy or photographs or printed matter that may be used in conjunction with the eventual use that it might be applied.

I understand that the finished copy or photographs or printed matter may be incorporated into any Prism Communications program or project.

I hereby release, discharge and agree to hold harmless Prism Communications its legal representatives or assigns and all persons acting under its permission or authority or those for whom it is acting, from any liability by virtue otherwise, that may occur or be produced in completion of the final product.

I hereby warrant that I am of full age and have every right to contract in my own name in the above regard.  I state further that I have read the above authorization and release, prior to its execution, that I am fully familiar with the contents thereof and understand that there will be no financial compensation for my participation or the use of my image.
